This is the Game Evaluation Form for NWISL. This form has taken the place of our Referee Evaluation Form. We believe, in order to create a better game experience for our families, and provide better training for our referees; we need to better understand the entire game situation as it happened.
This form must be completed each week by any Coach or Referee that wishes to win the Coach of the Season or Referee of the Season. There are links listed below for those forms.
