Request for Internet Access Assistance
If your family would like to borrow a T-Mobile Hotspot from the M-CV school district to assist with remote learning for your high school student, please complete the information below.  Once received, the high school will prioritize students and verify participation in the federal meal program, and then send you additional information to request the device.  All devices must be returned by  the beginning of August.

I am willing to abide by the district Acceptable Use Policy and the T-Mobile Hotspot Use Policy.  I am willing and able to pick up the Hotspot device and will ensure it's return to MHS by August 1, 2020.  I understand that acquiring a Hotspot may not be possible, but if it is, will require additional paperwork through the district. *
