4-Player Team Competition Sign Up
This form should be completed by the Team Captain ONLY. One form per team. All communication from the Event Officials will be with the Team Captain ONLY.
Teams consist of 2 Men and 2 Women. The Team Captain need NOT be one of the players.
ALL PLAYERS MUST REGISTER FOR THE 4-PLAYER TEAM COMPETITION EVENT TO QUALIFY TO BE ON A TEAM.


Division LIMITS: We'll be limiting each Division to teams in multiples of 4 TEAMS. The first 4 COMPLETE teams are secured in the competition. All other teams will be on a WAITLIST until we reach enough teams to add another 4 Teams. Only COMPLETE TEAMS will be entered into the competition.
