This form is in no way connected to the 4J Free and Reduced lunch form however it may provide you and your child additional benefits. If you have received an approval letter for a sibling, who attends a 4J non-charter school, please bring it in and we will make a copy. Students who qualify for free or reduced lunch can also qualify for reduced fees for community sports and recreation activities (see italicized information below from 4J’s website).

The summary information provides valuable data to be included on grant applications, fundraising publications and is shared with other charter schools and our sponsoring district.
