Hereby, grant permission to Mitchell’s Plain Music Academy and Kraaifontein Academy of Music to process the personal information collected on this registration form for purposes pertaining to the registration, teaching and assessment of the student indicated below. I also grant permission that photos/images/videos of him/her may be used for administration, promotion and marketing initiatives of the Music Academy including but not limited to:

• Video recordings;
• Fundraising proposals and feedback reports;
• Webpages and social media platforms;
• Classroom teaching activities, workshops, masterclasses and concerts;
• Printed publications such as newspapers, magazines, annual reports, programmes

In granting this permission, I understand that the Music Academy may use these images/photos/videos as deemed appropriate by the principal. I further understand that although the Music Academy associated with the images/photos/videos may be named, the name or other personally identifiable information of the child/student will not be used with and image/photo/video without prior consent.

I am signing this release form in the knowledge that any image/photos/videos posted on the music academy’s website could be downloaded and reproduced, and I therefore release the Music Academy from any liability arising from the use of images/photos/videos of the child/student in the Music Academy web postings.

I further understand that if I wish to rescind the agreement, I may do so at any time by sending a letter to the principal of the Music Academy.
