Complete this form to report your student's absence from school. The information that you provide will be sent to the appropriate contacts. 
  • Student absences should only be reported by the child's custodial parent or guardian.
  • In the comments section, please include an explanation for the absence.
  • Please be reminded that Melissa ISD students may receive a total of three (3) excused absences, by parent/guardian note, each school year.
