𝗣𝗹𝗲𝗮𝘀𝗲 𝗻𝗼𝘁𝗲: Registration is limited to 𝗣𝗿𝗼𝘃𝗶𝗱𝗲𝗻𝗰𝗲 𝗛𝗲𝗮𝗹𝘁𝗵 𝗖𝗮𝗿𝗲, Providence Research, staff from partnering health authorities, affiliated academic partners, and patient partners. Exceptions may be made by the organizing team on an individual basis.

Providence Research in partnership with St. Paul’s Foundation;  is using the information for registration purposes to manage Skunkworks Hacking Chronic Disease attendance. Providence Research is collecting your personal information through Google Forms under section 10(1) of the BC Personal Information and Protection Act.  

Registration through Google Forms is voluntary. Please be aware that IP addresses, event responses and personal email addresses (if supplied in place of a business email) will be collected by Google Forms and this information will be stored by Google Forms outside of Canada. By clicking on the Register button, you are consenting to your personal information (IP address, event response and email address) being stored in the United States. Your consent is effective as soon as you register. If you do not wish to register through Google Forms, please email (skunkworks@providencehealth.bc.ca) who will register you separately.
