General information
This section asks you to provide us with your full name and a means of contacting you.
Your legal name *
It can be kept private if you wish. This information will be used only for our internal records. Your workshop will be REJECTED if you do not fill out the field completely. Identification may be requested on-site.
Your usual name
If different from your legal name. This will be used for communication purposes. You may also mention your pronouns.
Your e-mail address *
Please provide us with the email address you use most frequently. Please ensure it is spelled correctly, as this is the only means we have to communicate with you.
Your phone number *
Please provide us with a phone number where we can reach you. This will be used only if we cannot reach you by email.

Information about your workshop
This section pertains to general information about your activity, as well as information about the presenters.
Name of your activity *
This is the name that will appear in the convention program.
The description of your activity (maximum 350 characters in length). *
This will be the description that we will include in the program. Please check the spelling to save us all time and headaches. Thank you very much!
What name (individual names or group name) would you like to appear on the schedule? *
Name of the co-host (if any)
Please provide the full names of the other hosts for this workshop, separated by a comma. A maximum of 2 will be allowed.

What is the appropriate age for attending and/or participating in your activity? *
This will help us choose the time for your workshop and inform the audience about the content of your workshop.
How long is your activity gonna be ?  *
Activities can be 1 hour, 2 hours, or another duration depending on the nature of the activity. A different duration will need to be discussed and approved per case as neeeded

Here are the technical questions regarding your activity, such as the required time and the equipment you might need or get cleared with security

Are there any events during which you will not be available? *
For example: masquerade, dance, early Saturday, late Sunday, etc.
Do you have any allergies or health conditions that we should be aware of?
For example: peanut allergy, heart issues, anxiety disorder, etc.
Are you bilingual and willing to host your workshop in both languages? *
We understand that you will primarily present your workshop in English. Some members of the audience may not be fluent in english and may request a translation. Will you be able to do this? Don't worry if you can't! It's just nice to know the possibility.
