Please prioritize with numbers 1 through 3 the statements that reflect the areas of ministry you would consider serving. Choose as many as you feel you would like the opportunity to serve. You may mark multiple choices with 1’s or 2’s or 3’s.

One’s (1) would indicate your primary preference(s) and three’s (3) your least. For example, you may enjoy prayer ministry and give it a 1, but being a Greeter may not interest you so you give it a 3.
