Administrative Policy: Comprehensive Review Extension Request

This form is to be used to request permission to extend the maximum length of time from the prior policy review period (currently 4 years) to five or six years. This form will be reviewed by the President’s Policy Committee and a determination whether or not to grant the request will be made.

Please complete the form and submit it with your comprehensively reviewed documents (policy, procedures, FAQ, etc.) at the time you’re submitting them to the University Policy Program Director for Policy Advisory Committee review.
