Purchase Request
This Uniform Order form is per child. Please only enter information for the items you wish to purchase. 

You will be invoiced via the Jackrabbit portal.  Please let us know if you have not been invoiced within a week of placing the order.

Payment must be received before collection.  Ensure the fit and colour are correct before taking home.  There will be no refund/exchange for items worn.

All orders take approximately two weeks to be delivered and are to be picked up in class from a staff member. 

Please take care with sizing - there may be sizes available at the studio for fitting.  As VPC does not hold excess stock and only collects orders weekly, incorrect sizing will cause delays.

Name Labels - 2 pack vinyl
Label your shoes immediately before the first class so someone else doesn't pick them up accidently.  Tip - put your labels in the shoe so it can be clearly seen when placed on the floor - under the heel works well.  Each pack $5. Dymo.  Please enter how many 2 packs required. Label will be in Child's name as entered in previous question.
Name Labels - material
Label your uniform pieces immediately before the first class for easy identification.  Each label $3.  Dymo. Please enter how many labels required.   Label will be in Child's name as entered in previous question.

A 15% discount will be applied if you buy one of the below packages. All items in the package must be purchased in one order for the discount to apply.

Jazz and Tap - Leotard, stockings, shoes and your choice of shorts or leggings 
Preschool and Tinies - Dress, socks, tap and ballet shoes
Ballet - Leotard, stockings, skirt and shoes
