PARENTS - I have received, read, and understood the contents of the Summer Creek High School Choir Handbook.  I agree to adhere to all policies, procedures, rules, and fees described therein.  These include, but are not limited to: student responsibilities and expectations, choir rules, eligibility policy, event attendance policy, grading policy, uniform information, fees, refund policy, and letter jacket policy.  I understand that failure to abide by this agreement may result in appropriate action including, but not limited to, teacher/student conference, teacher/parent conference, discipline referral, grade penalization (as stated in the grading policy), and dismissal from the SCHS Choir Program.

My signature verifies that I understand the significance of the responsibilities required of my student and of myself as outlined in the choir handbook.  I agree to support my student in his/her participation in choir, and to help foster the sense of pride that he/she can acquire through such participation.

Applicable for Chorale students - In addition to the expectations for all Choir students, I understand that as a member of the Varsity Choir, my student will be held to a higher standard.  I understand that if my student does not meet those expectations, he/she may be removed from Chorale and/or Colla Voce and placed in another Choir.  I understand that parent contact will be made in this instance.

During the course of the year, your student may be issued and/or be in possession of and/or use equipment* that is owned and maintained by the Humble Independent School District.  Humble ISD provides for repairs necessitated by the normal day-to-day use of the equipment at no cost.  However, the student’s parent or guardian may be held responsible for repair or replacement costs associated with theft, accidental loss, and/or damage due to neglect, carelessness, or abuse.

I understand that the above student and I, as parent or guardian, are responsible for the school equipment. I understand that we will exercise all reasonable care of it while under the student’s custody, care and control and accept financial responsibility for repair and/or replacement of such equipment as outlined in this document.

*These items may include music, folders, uniforms, garment bags, etc.

I give permission for my student’s photograph and name to be included in all Summer Creek High School Choir publications including, but not limited to, local newspaper articles, newsletters, choir website, SCHS website, and district publications.

I hereby give consent for my student to participate in and travel to curricular and extra-curricular activities associated with the arts education program in which they are a member and herewith grant permission for school employees to secure medical services, if necessary.  

I understand that the Humble Independent School District or its employee(s) do not assume any responsibility for accidents or illness that may occur nor are they responsible for associated costs which may be incurred during treatment of such.
