CDL Rapid Screening Consortium Privacy Notice and Consent (Students)

The CDL Rapid Screening Consortium (“Consortium”) and its members and participating organizations together with provincial and federal public health agencies are in the process of developing a system for conducting COVID-19 screening that can be administered on a cost effective basis and produce rapid results for the purpose of reopening the economy during the COVID-19 pandemic in the absence of widely available vaccines or treatments (the “Project”).

As a participant, The Mabin School will be operating a screening site (remotely under The Mabin School Operator’s virtual supervision - Thrive App), and will be administering screening to employees and students. Your child(ren)'s participation in the screening is completely voluntary. If you choose to participate, your child(ren)'s personal information will be collected, used and disclosed as set out below.

What personal information will be collected and used?

Registration Information: Full name, preferred contact method, contact information, gender, check-in and check-out times, screen start and end time.

Screening Results: Each time you are screened (remotely at home - twice a week, Tuesdays and Thursdays), screen administrators at The Mabin School will collect your unique ID, screen date, screen start time, screen type, screen result and screen reporting time.

PCR Test Data: Individuals that receive a positive rapid screen result will be required to undergo a COVID-19 PCR test and self-report their PCR test data to The Mabin School including PCR test date, location, result and CT Value.

With whom will personal information be disclosed?

Consortium: Certain registration and screening data will be disclosed to the Consortium, including: your role (employee, or student), age range, screen type, Site (at home), screen result, screening date, and self-reported PCR test result (if applicable). This information will be associated with a unique identifier assigned by The Mabin School which will enable the Consortium to link your screen results over time (“De-identified Data”).

The De-identified Data will be used by the Consortium to assess and compare the operational efficiency of the screening sites and different types of screens used throughout the Project, to ensure that screened individuals conform to the participation parameters of a given site, to evaluate the effectiveness of the COVID-19 screens, improving the operational performance of COVID-19 screening programs, to inform public health authorities on the use of the screens in practice and to measure if there is site-specific or general workplace issues with the screening program.

The Consortium will also use and/or disclose the De-identified Data to provide aggregated statistical reporting to The Mabin School, and in combination with information from other members and organizations participating in the Project, to (i) create and monitor best practices, benchmarks, and operational standards for Consortium members and participating organizations to allow participating organizations to assess and compare the efficacy of the use of the COVID-19 screens at their workplace; (ii) develop, enhance, and make available the Consortium’s services and products to Consortium members, participating organizations and other third parties, such as a specific dashboard for members and participating organizations, and (iii) produce and publish data, information, or other materials that are not identified as relating to a particular individual or The Mabin School.

The Mabin School may also share a copy of your completed consent form with the Consortium where necessary to validate your consent, such as in the event of a complaint, legal claim or request by a public health agency.

The Consortium will retain this information for the duration of the Project and will then anonymize the data so that it will not be possible to identify you using that data alone or in combination with other information.

The De-Identified Data will be stored on a confidential basis by the Consortium and its third-party service provider(s).
 
Public Health Agencies: The Consortium may also share De-identified Data with provincial and federal public health agencies to monitor the efficacy of the COVID-19 screening devices and programs, determine the prevalence of COVID-19, and facilitate safe and responsible employment practices to allow individuals to safely return to work. Provincial public health agencies may also use this data to inform their resource deployment in their combatting of COVID-19 and to assist public health care providers in detecting, preventing and treating COVID-19 infections in their regions.

Provincial public health agencies may also share this data with Health Canada according to their established protocols and in accordance with provincial legislation. Provincial public health agencies include Ontario Public Health, Alberta Health Services, Public Health New Brunswick, the Health and Welfare Commissioner of Quebec, and other provincial public health agencies, boards, arms-length corporations, and offices.

Participating Organizations: The Consortium will share aggregated data with other Consortium members and participating organizations for reporting and analysis purposes (as described above).

Consent

By signing this form, I acknowledge that I have read and understand the CDL Rapid Screening Consortium Privacy Notice and Consent, and agree to the collection, use and disclosure of my personal information, as described above, including the disclosure of my De-identified Data from The Mabin School to the Consortium, and from the Consortium to public health agencies.
