Thank you for your interest in becoming a Mentor to a teen in Follen's Coming of Age program. Please fill out your responses to the questions below to help our Coming of Age (COA) Youth get a sense of who you are. Youth will review submitted applications, then rank their choices for Mentors. Please be thoughtful in what you wish to communicate to them. Thank you.

Note: As per Follen's Safety policy, all COA Mentors must be 25 years of age or older, and will be required to pass an online background screening (currently provided by Trusted Employees, Inc.) before beginning work with Youth.
