Concordia Youth Action Consent Form

This form should be completed by a responsible adult over the age of 18, preferably the legal guardian. This form is best completed by the parent/guardian together with the young person who will be participating in the Concordia Youth Action programme.

The purpose of this form is to gather the information needed to keep your young person safe and support them during their time on programme. Please provide as much detailed information as possible. The information you provide will be shared with the group leaders, as part of the necessary administration of the programme.

Please confirm that your young person is aware you are providing this information on their behalf to Concordia, and that you give consent for the young person to participate in the Concordia Youth Action experience, by ticking the boxes and completing this form.

Concordia Youth Action - Introduction
The Concordia Youth Action programme supports young people in Sussex to engage with the local community. Underpinning all our sessions is the drive to develop young people’s skills and give them experiences that help them thrive. We align all our activities with the Skills Builder Framework, to help prepare for the next steps both inside and outside of traditional education.

The free Concordia Youth Action (CYA) Programme combines fun skills sessions and a social action project, resulting in an award. Activity sessions are aligned with the Skills Builder Framework, developing independence and life skills to support young people with education, employment and positive steps towards a brighter future.
