This form is for use by a parent/guardian in order to give consent for testing if their child who is under 18 years of age is identified as a close contact to a known COVID-19 case. Consent given below by a parent/guardian will remain in effect for the remainder of the 2022-2023 school year unless a subsequent form revoking consent is submitted.
