PARENTAL CONSENT FOR AN EDUCATIONAL VISIT

CHALFONT ST GILES JUNIOR SCHOOL – YEAR 5

Year 5 Cricket PE Festival at Chiltern Hills Academy, Chartridge Lane, Chesham, Bucks, HP5 2RG.

All of the children in Year 5 will be participating in a Cricket PE Festival at Chiltern Hills Academy on Friday 16th June between 1pm and 3:40pm (event time is from 1.40pm to 3.10pm). The children will be travelling to and from the festival by coach. Please note the planned return time.

Children will leave for the PE Festival at 1pm so will be able to have normal lunch.

Children should come into school wearing their school PE kit (green T-shirt, black shorts or black jogging bottoms and green hoodie) and trainers.

Children will also need a named water bottle to take with them.

We will need 3 parents to help on this trip. If you would like to help and have a current school DBS please contact the school office.

By completing this form I am giving consent for my child to attend the Year 5 PE Festival at Chiltern Hills Academy on Friday 16th June 2023. I confirm that I have read the relevant information above. Consent forms to be completed and returned by Wednesday 24th May.

I agree to his / her participation in the activities described and I acknowledge the need for and expect him / her to behave responsibly. I can confirm that the school has current contact and medical details for my child.
