I, being the parent/ legal guardian of the above child  (“the Dancer”), hereby acknowledge and agree that the Dancer is participating in auditions, workshops, rehearsals, shows and related events during the 2024/25 season (“the activity”). I am aware that the activity has inherent risks and that I have full knowledge of the nature and extent of the risk associated with the activity and that I give my full permission for participate in the activity. In doing so, I understand that participation of the Dancer is done so at the sole risk of the Dancer and the Society of Alberta Dance Theatre for Young People is not liable for any loss, damage, injury, sickness, ambulance service or other cost resulting from or in connection with such participation. I agree to indemnify and release the Society of Alberta Dance Theatre for Young People and its officers, directors, employees, choreographers and volunteers against all actions, lawsuits, damages, claims, demands or financial or other loss arising from items which may arise as a result of the dancer participating in “the activity”. 

I, parent/legal guardian of the above child [hereafter DANCER], grant permission to the Society of Alberta Dance Theatre for Young People [hereafter ADT], and its agents, employees or assigns, the irrevocable right to use any photographs and/or videos taken of and/or provided by my DANCER for use in any ADT related publicity such as advertising, direct mail, brochures, newsletters, social media and magazines, and to use the photographs on display boards, and to use such photographs in electronic versions of the same publications or on web sites or other electronic form or media. I also grant permission to ADT, its agents, employees or assign’s to offer the identified photographs and/or videos for use or distribution in other publications, electronic or otherwise, without notifying me. 

I hereby waive any right to inspect or approve the finished photographs and/or videos printed or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photographs. 

I grant permission for my DANCER to be involved in any on-camera, print or audio broadcast media interviews for the sole purpose of promoting ADT and/or it’s current show production. 

I am 18 years of age or older and I am competent to contract in my own name. I have read this release before signing below, and I fully understand the contents, meaning and impact of this release. I understand that I am free to address any specific questions regarding this release by submitting those questions in writing prior to signing, and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release.
