Information for D6 connect access
Information to correctly complete this form and install D6 connect
1. Examples have been provided in brackets to assist you.
2. All fields are required.
3. If you have more than 1 daughter at QGHS please do a new form per child.
4. If both parents wish to have access a new form must be completed by each parent per learner- so if Mom requires access and has 2 daughters she must complete the form twice and should Dad also require access he should also complete the form twice
5. Should a learner require access they may also complete the form, however, where parent details are required they will complete their own details
6. Please ensure you have the correct D6 app- we use D6 connect. 
7. Download from your PlayStore or Apple AppStore
8. The icon is dark grey to black in colour NOT Blue
9. Follow the prompts to install.

First name of parent (eg Elizabeth) *
Preferred name of parent (eg Betty) *
Initials of parent (eg E) *
Cell phone number of parent *
Email address of parent *
Grade of learner(s)- if multiple grades do a new form per learner *
Full name of learner (eg Amy van der Merwe) *
Submit
Clear form
Never submit passwords through Google Forms.
