A signed waiver on file is required for each student to participate in St. Andrew's Youth Ministry.

Typing full names and listing date signifies signing the waiver. Once signed, submit the form. Form will be kept electronically filed. Form will not be editable once signed. Please contact Jean Long with any concerns or questions regarding the waiver.
