In the event you received a notice that a student's device was not returned, and you have reason to believe this was an error, please fill out the following form so we may investigate the situation.

If the device is still in your possession, please return it to the Auxiliary Services Building located at 527 South 1600 West from 9-11 a.m.  Monday - Thursday, excluding Federal and State holiday observance dates.

Loss of equipment will be addressed in accordance with District Policy and Procedure.  Reference Policy 4202 P3.
