Your Grade Level Coordinators would like to thank you in advance for completing this very important questionnaire. The information you provide will be used for your letters of recommendation for college and scholarship applications. We ask that you be as honest and thorough as possible when responding to the questions and that you provide specific examples.
 
You do not need to fill out a Senior Questionnaire if you are only applying to UC, Cal Sate or other colleges and universities that do not require a counselor letter of recommendation.

You do need a letter of recommendation for most private colleges/universities. Most of the private schools will use the Common Application. Some scholarship applications also require a Counselor Letter of Recommendation.

We ask that you submit the questionnaire to your GLC by these DEADLINES:

Wednesday, September 15th, 2021
For Early Action (EA) and Early Decision (ED) Applications
If you are applying via (EA) or (ED) , include ALL of the colleges you are applying to (Including Regular Decision Applications) by this DEADLINE!
 
Friday, October 1st, 2021
For Regular Decision Applications

*Because of the numerous recommendation letters that we must write, we cannot guarantee that we will complete your letter by the due date if you submit a late response.

The Senior Questionnaire includes the following sections:

1. Recommendation Requests
2. Extracurricular Involvement
3. Academics
4. Personal Reflection
5. Parent Reflection (optional)

Lastly,  be sure to invite your GLC on the Common Application system.  We cannot submit a letter of recommendation unless you invite us through our email addresses:
