With registration of my child in the USC Girls soccer Boosters Summer Youth Camp, I hereby, for my child and me, waive and release any claim we may have for damages against USC Girls Soccer Boosters and Staff for any and all injuries that my child may incur as a result of participating in the above said program. I also certify that my child has been checked by a licensed medical doctor within the last year and is in good health. USC Girls Soccer Boosters also reserves the right to make decisions related to refunds for cancellations due to, but not limited to, an act of God or inclement weather.
