This Survey's intention is to gather information from RSU#13 families regarding Afterschool Programming needs you may have for school year 2020-2021. 21st Century is a FREE grant funded Afterschool Program open to families at South School.

We don't need any Afterschool care, academic support or enrichment. We have other plans for Afterschool.

We don't need any Afterschool care through 21st Century, my child(ren) attend another Afterschool/care program (like the YMCA or daycare program or other)

We do have a need for in person, free Afterschool care & programming through 21st Century and 3-5pm would work best for our family.

Transportation Needs Afterschool *

My student would need Bus/Free transportation home from Afterschool (at 5, 5:30 or 6pm) or they would NOT be able to participate otherwise

My student would participate and does NOT need transportation provide by the bus/program - WE will pick them up at 5, 5:30 or 6pm.

We wouldn't do Afterschool anyway, so this question is not relevant to our family
