G.I. Mutts Service Dog Application
G.I. Mutts finances the training of rescue dogs as services animals and matches them with soldiers and first responders suffering from PTSD and other conditions. For potential applicants for service dogs, we request the that you:  

​1. Have served in the military or as a first responder. We will ask for a copy of your DD214 or first responder ID once your application has been processed. If you need a replacement copy of your DD214, visit https://www.va.gov/records/get-military-service-records/

2. Have the resources to provide food, shelter, and care for a service animal.

3. Have the ability to travel to Oklahoma City to be matched with a dog. 

Once selected as a candidate, our team will reach out to you to better understand your needs. 

The need for service animals is greater than the supply. We work our hearts out to get as many dogs trained and matched as possible. 

This is our intake form for veterans and first responders interested in getting on the list for a service animal. There are three sections in this application: 1) basic information, 2) about your service, 3) about you and your service dog.
