Welcome to the WI AHEC Scholars Application for the 2019-20 Cohort! This application is open to students who are currently enrolled in either a health professions degree or certificate program that is a minimum of two years in length. Eligible students must be two years out from finishing their degree or certificate (i.e. students who just started their first year in a 2-year certificate or Masters degree program, students who just started their third year in a 4-year professional degree program).

This application includes 5 parts: (1) Applicant Contact Information, (2) Demographic Data, (3) Academic Information, (4) Career Goals & Interests, (5) Personal Statement.

Please note that in Part 5 you will be asked to respond to the following question and may want to prepare your response in advance before starting this application: In 500 words or less, please describe your background and career goals. How do you feel the WI AHEC Scholars Program will benefit you in reaching your goals?
