PhilCAFE or Philippine Coffee Advancement and Farm Enterprise is a five-year project being implemented by ACDI/VOCA and funded by the U.S. Department of Agriculture. It aims to increase the agricultural productivity of coffee farmers and increase the trade of coffee-based agricultural products.  It will work in 25 provinces across the country and will support 13,700 farmers and their families.  

INFORMED CONSENT TO PROVIDE INFORMATION:

In doing so, the project needs to collect information from partner individuals, farmers and organizations, specifically related to coffee farming, production, marketing, and consumption. By filling-out this form, please take note of the following:
1. All information provided will be treated confidential and will not be taken against the participants;
2. All personal information will be treated strictly confidential and will not be revealed in any of the reports;
3. No risk is involved as participant in this project;
4. You are entitled to refuse questions that you feel you should not respond to;
