Once you submit your information, Amy will review it all privately and contact you at the email address you provide at the top of this form. Please remember that if no valid email address is provided, you will not be contacted!

Amy receives submissions regularly and reviews them all herself. Your information will only be viewed by her.

**IMPORTANT: Submitting this form does NOT mean you are eligible to book as an Existing Client. Amy will notify you if your application aligns with her current scope of work and private level of support as well as if there is currently space within her calendar and 1:1 roster of clients. If Amy is unable to take you as a 1:1 client at this time, she will notify you of a timeline to which you will be able to book and place you on a waitlist.
