This Agreement is made between Good Shepherd Food Bank and the organization represented in the Cooking Matters Course Scheduling Form as it pertains to implementing Cooking Matters courses. A new agreement should be signed before each course series.

Attendance and Participation Expectation:
Cooking Matters is not a drop-in class program. Participants are expected to attend the full 6-session series. Participants are not allowed to repeat a Cooking Matters class series except when they did not graduate the first time. Cooking Matters requires at least 8 -15 participants per class.

Cooking Matters Agrees To:
Train volunteer instructors (culinary, nutrition, and support staff) to lead classes. This includes online orientation to the program. Provide funding for in-class groceries and take-home groceries (excluding Kids classes) to recreate at least one Cooking Matters recipe at home. Provide participant guides, graduation certificates, and all course paperwork. Provide ongoing technical assistance as needed.

Site Coordinator and Host Organization Agree To:
Your staff will be responsible for reminding participants weekly about class and having a presence in the classroom each week. Manage all Cooking Matters paperwork (surveys, attendance, gift card and receipts. All participants must sign a waiver, including waivers for participants under 18 signed by parents/guardians. Administer the Cooking Matters survey for all Adults, Families 6-week series, and any Parents and Caregiver series or Cooking Matters at the store.
