On the Back-to-School process portal, there are links to three documents that require your signature. The documents are Family Commitment Policy (which indicates that you and your child(ren) have read the applicable Parent/Student Handbook for new families, or the Major Handbook Changes document for returning families - also included in the Phase 1 documentation), Dignity and Respect Policy, and Technology Policy. Once those are read, please complete the student information and type your name at the bottom designating that you agree to and will adhere to Silverdale Baptist Academy policies.

This form must be completed by the first day of school, August 16th.
