Lewis HS: Request Form to Change from 2 Days In-Person Learning or Virtual Learning to 4 Days In-Person Learning

Parents are invited to fill out this form to express their interest in having students attend school In Person four days (Tuesday-Friday) per week. Requests will be examined in the order in which they are received. This form will be available in April only; requests will not be accepted in May or June. Once the request is received, we will take a look at the student schedule. If we are able to grant the request, the parent and student will be contacted via email with the approval and next steps. This process can take up to 7 business days to process. We thank you for your patience, and look forward to continuing to see your children at our school!
