This form should be used for:

- Boys aged 8 to 21 - (training for 8-13 year olds Tuesdays 6.30pm from May 7th)
- Boys under 14s and above  - (Training is on Mondays from 6pm from May 13th)
- Girls aged 11 and above (Tuesdays 6.30pm from May 7th)

**Cost will be £70 - and will include a cap and white playing shirt**

By submitting this form  I consent to : 

In the case of injury, accident, or illness whilst in the care of Harold Wood Cricket Club that a Club Officer or Adult Member of the Club may give permission on my behalf for the carrying out of ANY medical treatment, including blood transfusion, and surgery on my child as considered necessary by a qualified medical practitioner.

My Child being photographed in cricket situations in line with the recommendations as set out in the Club’s Photography, Filming and Social Media Broadcasting Policy

My child, if playing in an Adult team, may change in the same changing room as the Adult players.

That the Club may publish photographs and articles on the Club website that may include or reference my Child. (Note: You may at anytime request any photo your child is in to be removed from the website or any article to be changed so any reference to your Child is removed or amended)

Grant permission for my child to participate in all competitions including home/away fixtures & events

By submitting this form I hereby agree to : Indemnify Harold Wood Cricket Club, its officers, coaches, managers and volunteers, from any liability for loss or damage to personal property, or accident or injury to my Child which occurs whilst they are receiving coaching, playing cricket matches or participating in any club event or activity.

My Child complying with the Club’s rule that all Club Members under the age of 18 playing any form of hardball cricket (incl. Net practice) will wear a helmet when batting or keeping wicket in a match or training.

Being contacted through the club communications channels with regards to activities at the Club (e.g Teamer - for colt match selection of your child, WhatsApp Broadcast for information regarding the Club activities, text/messages etc)
