2022-2023 EIS Schedule Change Request
This form is ONLY for students who have an error on their schedule (missing class, duplicate class) or are in the incorrect placement (e.g. math level, GT, Band/Choir). Once schedules have been posted on Family Access, requests can not be made for specific lunch times, teacher requests, friend requests, etc. By submitting this request I understand that this change may affect my child's other classes and teachers.
