Hello!  Welcome to your Sweet Style Nails Monthly Subscription Box!!!  Please fill out the form below to get set up! Let me know if you have any questions or suggestions!!! Please answer the questions as completely as possible.  Every price already includes shipping!!!  

As a BONUS, you will automatically receive a free set and small gift for your birthday, so make sure to fill out your birth month and day!!

Invoices will be sent out by the 5th of each month and need to be paid within 48 hours. Boxes will be shipped out by the 10th of each month.  Your subscription can be changed or cancelled anytime.

Your subscription will be automatically renewed but if you need to skip a month, simply do not pay the invoice! BUT as another BONUS after every third consecutive month you pay, you will get free shipping your next month (i.e. free shipping on the 4th, 8th, 12th months and so on). If you need to cancel your subscription, just put in your name and skip to the bottom and select "Cancel my Subscription."
