Omnipod 5 Pre-work

You have selected the Omnipod 5 and we look forward to supporting you.

There are tasks detailed below that need completing before we can book you to start.

The tasks will take you an hour to complete and must be done with the person who has diabetes if they are over the age of 8 years old.

We cannot order the pump or book the start until the tasks have been completed.

If you cannot compete the tasks email us at Contact us and we will set an appointment to guide you.

If you get more than one question wrong for the video questions, you will need to re-take the questions.

The questions are easy if you watch the full videos.

The score will be revealed on completing the form.

We will be in touch soon with next steps or support if you are unable to complete some of the tasks.
