Same day evaluation fee, $30.00 - Cash accepted.

Team evaluations are based on athletes age, skill level and coachability/experience.

Athletes may be asked to return for stunt callbacks Sunday 5/19.

In consideration of being allowed to participate or attend an event, in any way at Phoenix All-stars, LLC/East Celebrity Elite Manchester athletic/sports programs, related events and activities, the undersigned acknowledges, appreciates, and agrees that: There is risk of injury from the activities involved in this program. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of staff employees, Phoenix All-Stars, LLC/East Celebrity Elite Manchester, or others and assume full responsibility for my participation and that of my minor child or ward for whom I am signing for as legal guardian of a minor. This is to certify that I, with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the releases and I further certify that I have health insurance coverage on myself, or my minor child/ward, and the coverage will remaining in full force and effect during the period I, or my child/ward remains enrolled with Phoenix All-Stars, LLC/ East Celebrity Elite Manchester, I understand that the failure of Phoenix All-Stars, LLC/ East Celebrity Elite Manchester to verify this information does not waive my responsibility to comply.
