Incoming Preschool students MUST be four years old before August 1, 2023.
All screenings are by appointment only.
Families will need to bring the following information to the screening:
● Birth Certificate/Proof of Age
● Shot Record
● Proof of Residency
● Insurance Card
● Custody Papers (if applicable)
Within 2-3 days after your registration has been submitted a school employee
will contact the family to set up a screening appointment for students.
Preschool Screenings will take place at the Dallas County R-1 Early Childhood
Center on the following dates
March 28th- 8:30-12:00, 1:00-4:00, 5:00- 7:00
March 29th- 8:30-3:00
**Completing this form does not guarantee enrollment. Your child is not enrolled in our district until all forms are completed and required documentation is supplied and approved. If you have any questions or have problems completing this form please contact the Mallory Office at 417-345-2350.
