I hereby acknowledge the health of my son/daughter to be ready for vigorous activity and authorize the camp directors and coaches to secure any emergency treatment deemed necessary, and that I hereby release the camp directors, coaches, and school from all claims of account of any injuries or loss of property which may occur while attending the basketball camp or in transport to the campsite.  If there are any questions or special information about your son/daughter that we should know, medically or otherwise, please inform us.  Thank you.
