Hello! Please fill out the following form if you intend on continuing to mentor the high schooler you were matched up with LAST YEAR. This form is for our records to know that you will continue mentoring the same student for the 2021-2022 school year, and if you do not complete it, we will assume that you stopped mentoring with the program in May 2021. Attending our first GBM this year is mandatory to continue as a mentor. By submitting this form, you agree to the mentorship rules outlined last year at previous meetings and also agree to attend the first meeting this year to review these rules. If you cannot attend the meeting, you must view the recording and slides posted to look over what your duties as a mentor are. Do you agree to these terms? (Only those that have reviewed our first meeting slides are eligible to participate as a mentor) *
Do you understand that if you can no longer mentor someone, or if issues arise with your mentee, it is your responsibility to inform the board promptly of this via email? *
By submitting this form, you are agreeing that you have understood the mentorship rules outlined at the first GBM (09-14-20) and will abide by them. You also acknowledge that any inappropriate behavior will not be tolerated and will be grounds for immediate termination as a member of this club. Further action will be taken if necessary. *
If you have further questions about mentorship after reviewing our Info Session Slides/Recording (08-31-20) and our GBM 1 Slides/Recording (09-14-20) from last year, please be sure to review the slides from GBMs 2-4 as well that were posted last year.
