Thank you very much for your participation in Onmyoji Giveaway Event. This form is for collecting the necessary information for claiming gift.

It will take about 3 minutes to finish the questionnaire.

Things to Note

1. All the information filled in the form should be accurate and based on the truth. Any changes after submission will be declined. Under no circumstance would any imposter claim gifts under false pretense be allowed.

2. The form is for collecting information of those Lucky Onmyoji (the winner) for the Facebook giveaway event

Onmyoji will comply with laws and regulations related to the protection of personal information, to use and manage the submitted information appropriately and cautiously, and delete such information immediately after issuing the gift.

3. If the winner fail to submit the correct information leading to a situation that Onmyoji could not contact the winner for issuing the gift, the winner would automatically lose the right to reclaim the gift.

4. The substantial gift will be delivered by the cooperative logistic service provider. Since the Coronavirus Pandemic is still ongoing in many countries and regions, the delivery time might delay due to the force majeure. Consequently, the winner are not allowed to designate the specific date, time or service provider.

5. Please kindly note that the issue of gift may take time to run all the workflow (especially gifts of high value will take more than a month of workdays). Please wait for our update by【e-mail address ❗❗onmyojiencommunity@163.com❗❗】after you filled in the form.

6. Please do keep in mind that we will not ask for your *passwords* of your bank accounts or *passwords* of digital payment accounts. We will not ask you to register in skeptical third party website either. Thank you very much.
