INTERNSHIP AGREEMENT FORM: SUMMER 2024

The prospective site supervisor should fill out this form and briefly lay out the expectations for this internship. Internship sites should demonstrate the capacity to provide a full-time quality learning and career-advancing experience that is well-supervised, safe, stable, and enriching for the student, for 8-10 weeks in summer 2024.

UC Santa Cruz students are eligible to apply for Transforming Futures, a scholarship program to support summer internships. The scholarship program is administered by the Institute for Social Transformation at UC Santa Cruz.
