Please complete this form by 11:59pm on September 24, 2021 in order to attend the conference at Mt. Lebanon High School on November 5 & 6! The Registration fee payment is due by 11:59pm on October 8, 2021.

IMPORTANT: Google forms do not save your progress once you exit the tab or browser. The entire registration process will take about 25 minutes to complete. All information that you will need to fill out this form is listed on the printable Registration worksheet, which is linked on the Registration page of our website. We strongly recommend that you review the printable Registration worksheet before you begin the Registration Google form so that you can complete the Google form in one sitting.

In order for your registration to be complete, you must do the following by October 8, 2021:

A confirmation receipt will be sent to the advisor upon receiving ALL of their school's registration materials and payment.

Advisors: In the case that you and your delegates are no longer able to attend the Conference due to COVID restrictions, refer to the PASC policy in the link below. Please note that if a student is no longer able to attend, you can decide to replace that student with another student from your school and the second student will not have to pay an additional Registration fee. The new student must complete this Registration form and it is imperative for the Advisor to notify the Registration Chairperson (pasc2021maddy@pasc.net) to inform her of this change.
