My child/ren will participate in activities with Hamline Church this year! This form is required for children participating in Hamline Church activities for families, whether in person or on-line. It provides us with accurate information about how to contact you; it also helps ensure that we can provide your child with excellent care in the case of an emergency. This form must be completed each year that a child remains enrolled in Hamline Church Programming.

After you enroll one child, you will have the option of enrolling others.
