Baby and Sunshine 2020 Super Box Giveaway Nomination

Please use this form to provide as much information as you can to help us decide our Super Box winners for this giveaway.

Qualified nominees are moms with baby(ies) between 0-24 months, having been affected by one or more of 2020's disasters (fires, hurricanes, unemployment, etc.) Winners will be receiving a total of one (1) Mom and Baby Super Box.

We understand in some situations, our winners may have evacuated and potentially not have a ship-to address. In such cases, please provide us with an alternative reliable address that we can send a large box to so that our winners can receive their box.

We are accepting nominations from 9/9/2020-9/30/2020 and boxes can start shipping out as early as 9/16/2020.

If you are a winner, we will contact you via the email address you have provided. If we do not hear a response back to confirm your application and that you'd like to move forward as a winner within 48 hours of the email being sent, we will go ahead and select a different winner to take your spot.
