The Perkiomen Valley School District Hall of Fame was established in 2018 to recognize distinguished alumni or others whose outstanding athletic, professional, or civic endeavors have brought honor to the district and inspire future graduates to perform and serve in the same spirit. Thank you for submitting a candidate for consideration. Please submit this form by March 31, 2023 for consideration of induction in October 2023. Please provide as much detail as possible to support your nomination. Lack of sufficient information may lead to your candidate being removed from consideration.

When completing this form, please keep in mind the following criteria:

* Nominee has displayed exemplary achievement in the areas of athletics, citizenship, community/military service, academic and/or professional achievements post-graduation.
* Nominee has earned recognition, honors and awards post-graduation.
* Nominees do not have to be PVHS graduates, but if they are, they must be out of high school five years or more to be considered as a candidate. A PV graduate is defined as someone who graduated from Perkiomen Valley High School following the incorporation of the school district in 1969.
