The Athletic Training Department has arranged for athletes 7th - 12th grade to get their 2019- 2020 athletic physicals completed by Laurens Family Medicine at the Laurens District High School gym on Wednesday, May 22, 2019 starting at 5:50 pm. The cost of the physicals is $20.00 cash only.

To ensure that a physician sees you, please select an appointment time.

***CHANGE FROM LAST YEAR*** No athlete will be allowed to see the doctor until ALL PORTIONS OF THE PHYSICAL IS FILLED OUT BY PARENT. Meaning, the front of the physical has to be filled out (signatures, answers to questions, etc.) BEFORE they are allowed to see the MD. This allows for a more smooth experience with the physician.
