Please read the following and sign below to indicate your understanding and agreement: The Canning & District Recreational Commission staff, including those contracted by Canning & District Recreational Commission (referred to as “staff”) is responsible for the supervision of participants during the above registered program(s), which has inherent risks. The staff reserve the right to remove any participant from the program if their behavior is such that it interferes with the safety of themselves or others, or if the operation of the program is hindered. I, the undersigned, do hereby agree to participate in the stated registered program, and I further agree to indemnify and hold Canning & District Recreational Commission staff harmless from and against any and all liability for any injury which may be suffered by the aforementioned individual arising out of or in any way connected with his/her participation in this program. I further authorize the administration of any first aid steps that may be deemed necessary by qualified personnel. *
My understanding is that the class instructor has the right to stop me from doing exercise which they feel would be harmful to me or make me stop exercising upon observation of any symptoms of distress or abnormal response. *
My obligation is to immediately inform the class instructor of any unusual pain, discomfort, fatigue, or any other symptoms that I may incur during or immediately after physical activity. *
I understand that this is a child friendly fitness environment; I am responsible for my child for the duration of class and acknowledge that child care is not provided.

Videography/Photography Consent: we occasionally take photos or videos to document programs or activities which could then be used in our educational or promotional materials. Permission is granted for CDRC to use still photographs or videos for this participant for these purposes only. *
I understand that completion of this form does NOT guarantee registration for this program; CDRC staff will follow up to confirm registration and receipt of payment (if applicable) once information has been reviewed and verified. I agree that I will not attend programming until confirmation of registration has been received. *
