NPSC Refund/Credit Form

Thanks for clicking through and thank you for being a part of the New Providence Soccer Club.  We hope everyone is healthy and safe.  As you know, the leagues in which our NPSC teams compete have recently canceled the Spring season so as a result we'll be canceling all Spring soccer activities.  

We're offering the option of either a full refund (in the form of a check, delivered to your home) or a credit (in the form of a coupon code) for the Fall season.  Please fill out the form below to indicate your preference.  

A possibility remains for some Summer alternatives for the club and these leagues, but that will be determined at a later date.  We'll keep you informed if Summer soccer can happen.  We will also be in touch to communicate details for Fall try-outs and registration once we have a clear picture on any type of "return to play" scenarios.

Please submit this form for only ONE child at a time.  If you have more than one child enrolled in NPSC Spring soccer, you can click "Submit" at the end of this form and the next page will have a link to "Submit another response".  Click that link and fill out a new form for each child.

In all of our soccer seasons, a portion of your fee goes toward purchasing necessary equipment for our club.  Equipment like field lining paint, goals and nets, pinnys and flags.  If you would like to donate a portion of your refund or credit to the NPSC to ensure we are able to buy the equipment we need to start the Fall season, please click one of the options below.  Thank you!
