Make Up Slips Request
LTS Families, 

Please fill out the form below to request your make-up slips. We will verify the dates listed based off of the coach's attendance sheet. All make-up slips will be ready for pickup within 24 hours of form submission. 

Reminder: Make-Up slips in the Fall/Winter/Spring may be used during Make-Up Week - a calendar of events is available on the LTS page on DSC's website. Skater must be enrolled in order to use make-up slips for Learn to Skate classes. 

Make-Up slips can be used for Public Skating without expiration.
