Nomination Form - End of Year Recognition Dinner for Office of Diversity and Inclusion
All members of the Earlham community (faculty, staff, students and alumni) are invited to submit nominations in the following categories. Winners will be announced and recognized at the End of the Year Recognition Dinner on April 24, 2019.

The deadline to submit a nomination is Friday, April 19, 2019.

Nominations are currently being solicited in the following areas:

Organization of the Year
This award recognizes a registered club that has demonstrated excellence in embodying the guiding principles of the Office of Diversity and Inclusion: social justice, collaboration, community, learning, intercultural competence, leadership, and support during the 2018 - 2019 academic year. This club must have made a meaningful contribution and positive impact on the Earlham student experience and the greater campus community during the year.

Program of the Year
This award will be presented to a registered club or student whose program strives to highlight the unique and diverse characteristics of Earlham’s student body. This program should educate the campus community through the provision of cross-cultural activities aimed to enhance community awareness and intercultural dialogue around diversity and inclusion.

Outstanding Faculty/Staff
This award is given to a faculty/staff member who is admired and respected by students for their outstanding dedication to advocating for diversity issues both in and out of the classroom. The recipient of this award has excelled in leadership, demonstrated concern for and sensitivity to the needs of multicultural students (above and beyond their job responsibilities) and is dedicated to the students at Earlham College.

The Rising Star Award
This award will be presented to a multicultural student (freshman/sophomore/transfer) who has demonstrated a passion for increasing diversity and inclusion through co-curricular involvement and leadership.  This student exemplifies the potential to be a role model, effective communicator and student advocate for the rest of their tenure at Earlham.      

The Impact Award
This award is presented to a multicultural junior or senior who has demonstrated cultural competence and a commitment to advancing diversity and inclusion across their undergraduate career. Nominees should serve as a role model, advocate for underrepresented students, and collaborator, working with partners across our campus and community.      

Grit Award
This award is given to a multicultural senior who might not be acknowledged for traditional standards of achievement like GPA, but has undoubtedly strived for excellence. They are intrinsically driven and take personal responsibility for their learning even during difficult times. This student consistently works hard and takes their academics seriously.
